25+ Local Offer
We want you to know that once you turn 25, we are still here.
- If you have remained engaged with the service you will have an up to date final plan completed within the 6 months prior to you turning 25.
- You will be written to prior to turning 25 with a “next steps” letter which will contain relevant local contacts for you.
- We will celebrate turning 25 with you if you remain actively engaged in the service, this should not be seen as a “good bye” but a celebration of achievement and an opportunity for reflection on how far you have come.
Ongoing offer of support
- We will continue to be a point of contact for you at any age, we can offer you with advice and guidance and signposting to relevant services.
- Access to weekly multi agency drop in sessions on Thursday afternoons from 1-6pm at our Oasis Hub. This is via a pre arranged appointment, to include,
- Education employment and training
- Specialist health services
- DWP
- Wolverhampton homes
- UASC champion.
- A Monthly “cake and catch up” drop in will be held on the first Wednesday of the month 10-12pm at our Oasis hub offering a more flexible drop in and catch up. Pop in and tell us how you are getting on.
