When you are in care you will have something called a Care Plan. A Care Plan tells you how you will be cared for and supported while you are in care, and help to make decisions for your future.
Your social worker will discuss this with you to find out your wishes and feelings before writing the care plan and make sure that you have your say. You should have a care plan within 10 days of coming into care. You will be given a copy of your care plan. We will listen to what you say about how you wish your care and support to be provided.
When it is written down it can look very complicated, your carer, social worker or another trusted adult can go through it with you to help you understand. You care plan says what everyone involved will do to work with you and support you.
